Create a hole with a masonry core drill that is at least 1 inch larger than the diameter of the railing or bolt. Here you use the mentioned epoxy type anchors. Be sure to have plenty of extra bits on hand and use a can of compressed air or a blower to clean. The hole should be from 2 inches to 4 inches deep. Place flange on concrete anchors.
For weak concrete i found that misusing the drill to create a bigger hole diameter in the bottom part gives added strenght but for this the anchor should be at least 10cm deep in the concrete thin 5cm slabs can t be fixed with epoxy anchors. When installing these concrete anchors special care must be taken to ensure safe and proper fastening. Large diameter tapcons or ldt s are the fastest strongest fasteners i ve found for anchoring to concrete.
